Whether you're managing near misses, injuries, illnesses, spills, property damage, or vehicle accidents, Serenity provides the tools needed to meet regulatory requirements and promote a culture of safety. ## What is considered an Incident? An EHS Incident is any event that has caused harm, or had the potential to cause harm. Examples of these types of events include: - **Injuries & Illnesses** - **Near Misses** - **Chemical Spills or Releases** - **Property or Equipment Damage** - **Vehicle Collisions or Damage** These events can be reported by **any system user**, ensuring that employees at all levels can help maintain a safe work environment without requiring advanced permissions. ## Incident Management Workflow in Serenity ### Reporting Incidents Incidents can be reported directly from the Serenity Home page using the **Report an Incident** button, or from the **Incident & Investigation Management** area in the left navigation. A short description and an establishment are required. You can also add a description, people involved, zone, exact location, dates, assignment, and categories. A custom intake form designed specifically for your workforce is also available. ### Reviewing and Managing Incident Details Once submitted, assigned users are notified and can begin reviewing the incident. Find incidents under **EHS incidents** in the Incident & Investigation Management area, using lists such as Opened by you, Assigned to you, Assigned to your group, Pending your review, and All incidents. On the **Incident Details** tab, each incident may include related detailed reports, which you add with buttons such as **New injury/illness report**, **Add release**, and **Add property damage**: - Injury/illness reports - Release reports - Property damage reports - Vehicle damage reports EHS users must review and finalize these records before the incident can be closed. Fields required for OSHA compliance are flagged to help ensure complete documentation. The **Follow-Up & Actions** tab of the incident lets you add witness accounts and create findings and corrective or preventive actions. Select **New**, then choose **Action** or **Finding**. ### Investigating Incidents For high-severity incidents or those involving injury/illness, a formal investigation can be launched directly from the incident record using the **Launch formal investigation** button. This creates a separate task where investigators can: - Collect and review evidence - Perform root cause analysis - Assign and track corrective/preventive actions - Generate an investigation summary This separation ensures clarity between the incident record and the investigation process. ### Closing an Incident Each organization sets its own closure policies. Some may close incidents after finalizing reports, while others keep them open until investigations or follow-up actions are completed. To close an incident: - All associated reports must be finalized. Select **Close Incident**; if any reports are still open, Serenity lists them and asks you to complete them first - Users can optionally request a review before closure - Reviewers receive an email notification to approve or request changes ## OSHA Recordkeeping Serenity supports OSHA Forms **300**, **300A**, and **301**: - Form 301 can be generated from any injury record - Forms 300 and 300A are created from each establishment’s **OSHA Reporting** section. Select **Start a report** and follow the steps: Reporting Year, Employment, Injury Totals, and Prepare reports The system auto-calculates metrics like: - Total recordable incidents - Lost time and restricted days - DART, TRIR, and LTIFR Users can download OSHA logs at any time, and Serenity ensures these reports reflect finalized and properly classified injury records. ## Tracking Hours Worked Accurate OSHA reporting and KPI calculation require monthly entry of **hours worked**: 1. Open **Organization Management** in the left navigation and open the establishment 2. Select **Time cards** in the left menu of the record 3. Select **New time card**, then enter the date, employee type, and hours, and select **Add entry** 4. Select **Submit** to save the entries for the reporting period Users can enter hours for multiple employee types (e.g., employees, contractors) to ensure precision. For greater efficiency, Serenity offers **integrations with payroll and timekeeping systems** like ADP, Workday, and SAP. These integrations are available via our **professional services team** and help ensure your safety metrics and reports are always up-to-date. ## Roles & Permissions Access to incidents and investigations is controlled by the roles in the Environmental, Health, & Safety category: - **User:** view and edit open incidents and investigations that are assigned to them or to one of their groups - **Manager:** view all incidents and investigations and edit those that are in an open state - **Privacy Case User:** view and edit injury reports that have been classified as a privacy case - **Admin:** full access to view, edit, and delete all incidents and investigations, and reopen closed records Roles can be assigned to individuals or groups. Best practice recommends assigning them to groups for easier maintenance. See Roles & Permissions for the full list of roles. ## Dashboards & Insights Incident and injury data feed the EHS Incidents Dashboard and the EHS Injury Trends Dashboard. Open **Dashboards** in the left navigation and use the dashboard selector next to the title to switch between them. The EHS Injury Trends Dashboard provides: - KPI tracking (DART, TRIR, LTIFR, and LTIR) - Injury counts such as recordable, lost time, modified duty, and reportable injuries - Breakdowns by nature of injury, injury mechanism, and treatment provided - Views for the last 12 months, this year, last year, and trends over time, with filters for Org Unit, Establishment, and Employee Type **Note:** Injury data and metrics refresh nightly and may not reflect changes made today. Dashboards give EHS leaders and executives the visibility they need to monitor safety performance and drive continuous improvement.
