# Recommendations Stage

The **Recommendations** stage is the final step in the investigation workflow, where investigators document findings and define actionable steps to correct and/or prevent recurrence of the incident being investigated.

## Transitioning from Root Cause Analysis

Once root cause analysis is complete—whether using the **Fishbone** or **ECF (Event & Causal Factor)** methodology—users can progress the investigation by clicking the **“Draft Recommendations”** button at the top-right of the page.

Doing so will:

- Advance the investigation to the **Recommendations** stage
- Automatically generate **Findings** for causal factors that have **Create finding** checkbox checked.

## Findings

Each **Finding** represents a root cause or significant problem identified during the investigation. These can be automatically created from causal factors or manually added by clicking the **New finding** button.

When adding a finding, users can:

- Provide a short and detailed description
- Assign it to an individual or group
- Set a due date, state, and priority

## Corrective & Preventive Actions

Each finding can have one or more **actions** assigned to address it. These can be:

- **Corrective** – to address the current issue
- **Preventive** – to prevent similar issues in the future

To add an action, click **+ New action** within a finding. You’ll be able to:

- Describe the action
- Assign responsibility
- Set due date, type, and priority

## AI-Powered Suggestions

Need help generating actions? Use the **“Generate new actions using AI”** feature to receive intelligent recommendations tailored to the identified finding. AI suggestions include:

- A short action title
- Detailed rationale
- Suggested priority and type

Review and select the ones that are applicable—then hit **Submit** to add them directly to the finding.

## Wrapping Up

Once all findings and actions are documented, the investigation can be formally **closed** by clicking the **Close Investigation** button.

This stage ensures that all root causes are not only identified but are met with targeted, trackable actions—making this a critical part of your continuous improvement and compliance processes.
