Closing a Finding Record

Once a finding has been investigated and the necessary corrective or preventive actions have been implemented, it may be appropriate to close the finding. Serenity provides a simple and secure process for doing this, ensuring both traceability and accountability.


How to Close a Finding

Step 1: Open the Finding

Navigate to the finding record in the Findings & Actions center. You will see the “Close Finding” button in the upper right-hand corner of the screen.

Step 2: Initiate the Closure Process

Click Close Finding. This opens a modal window that prompts you to complete a few required fields before proceeding.

Step 3: Provide Closure Information

You will be asked to provide:

  • Remediation Code
    Choose one of the following:

    • Remediated – The issue was fully addressed.

    • Will remediate – Remediation will be handled separately or is in progress.

    • Will not remediate – The issue will remain unresolved (usually requires justification).

  • Close Notes
    Provide a summary of what was done to address the finding or why the decision was made to close it.

  • Request Review (Optional)
    Check this box if you would like one or more users to review the finding before it is officially marked as closed. If enabled, you’ll be able to designate reviewers

Step 4: Sign Off

After entering the required information, click Continue. You’ll then be prompted to certify the closure with your digital signature:

  • You may type your name or draw your signature.

  • Check the certification box stating that the information is true, accurate, and complete.

  • Click Sign and submit to proceed.




What Happens Next?

If No Review is Requested:

  • The finding is immediately marked as Closed Complete.

  • The closure is logged with your signature and timestamp for auditing purposes.

If a Review is Requested:

  • Reviewers will receive an automated email notification with a link to the finding.

  • Each reviewer can:

    • Approve the closure, moving the finding to Closed Complete.

    • Request Changes, returning the finding to a Work in Progress state and automatically logging their review comments in the work notes.

  • Always ensure all related actions are completed or appropriately deferred before closing a finding.

  • Use the review feature for findings that are high-risk or require additional oversight.

  • Keep close notes clear, concise, and evidence-based to support auditability.
