Serenity makes it easy for users to report incidents directly from the homepage — helping your organization respond quickly and consistently to safety events.
How to Report an Incident
To begin reporting an incident:
Go to your Serenity homepage
Click the “Report an Incident” button at the top of the page
This will open a modal form (as shown below), where you can complete all required information.

Here’s what you’ll need to fill out:
📤 Manual vs. Automatic Assignment
You have two options for routing the incident after submission:
Manual Assignment: Fill in the “Assignment group” or “Assigned to” fields
Automatic Assignment: Leave these fields blank — the system will apply your organization’s assignment rules (typically based on Establishment)
💡 Unless you have a specific person or team in mind, it’s recommended to leave assignment fields blank and let the platform route the incident for you.
Once Submitted…
After clicking Save:
The incident is created in the system
It will be automatically routed (if not manually assigned)
Assigned users/groups will be notified via email
You’ll be able to track the incident’s status and updates
Who Can Report an Incident?
All Serenity users — regardless of role — can report incidents. This ensures everyone in your organization has the ability to document safety events quickly and accurately.
Custom Incident Report Forms (Optional)
If your organization has opted for custom intake forms as part of your Serenity implementation, you’ll see those forms displayed just below the welcome banner on your homepage — under the “See something? Say something!” section.
These custom forms are designed to simplify the reporting process for employees who may not be familiar with EHS systems or technical terminology.
What Makes These Forms Different?
Unlike the standard “Report an Incident” modal, custom intake forms:
Present questions in a plain-language, easy-to-understand format
(e.g., “What happened?” instead of “Description”)Combine multiple data types — like injuries, spills, and property damage — into a single, streamlined form
Can use dynamic logic to show only relevant questions based on user input
Are tailored to your organization’s specific terminology and processes
Customized for Your Team
Every intake form is built specifically for your organization. That means:
The questions and layout will reflect your internal policies and reporting standards
The fields shown may vary from site to site or role to role
The form can guide users through the entire incident submission process without needing training

Not Seeing the Forms?
If you don’t see custom forms under your welcome banner, your organization may not have this feature enabled yet. Reach out to your System Admin or Serenity support contact to learn more about whether this is available to you.
