Once an incident has been reported, it enters the reported stage, where EHS personnel or assigned groups verify, update, and document all the related details. This step is crucial for ensuring accurate recordkeeping, regulatory compliance, and proper follow-up actions.
Assignment Notification
As soon as an incident is assigned, the designated user or group will receive an email notification with:
A summary of the incident
A direct link to open the incident record in Serenity
💡 Users can also view their assigned incidents at any time by selecting “Incident Center” from the left-hand application navigator.
Incident Record Layout
When viewing the details tab of the incident page, the layout is split into two key sections:
1. Incident Details Panel (Left Side)
Includes the core attributes of the incident:
Short and detailed descriptions
People involved
Date of occurrence and reporting
Assignment details
Severity and categorization
Location and environmental conditions
This information is typically populated during the initial report, but can be edited or expanded during review.
2. Related Reports Panel (Right Side)
Displays any associated reports related to the incident:
Injury & Illness Reports
Spill & Release Reports
Property Damage Reports
Vehicle Damage Reports
Each section shows a status indicator (e.g., In progress, Complete) and allows users to view, continue editing, and finalize the associated reports.
📌 These detailed records are critical for regulatory reporting and may feed into OSHA logs or internal dashboards.
Requirements Before Closing
The system will not allow an incident to be closed until all related reports (injuries, spills, property or vehicle damage) have been reviewed and finalized by the assigned user or group
This helps ensure that nothing falls through the cracks and that your documentation meets internal and regulatory standards.
