When an incident involves an injury or illness, Serenity provides a structured injury report form to ensure accurate, complete documentation in line with OSHA recordkeeping requirements.
The system is pre-configured with required fields for compliance purposes, but this configuration can be customized by Serenity’s Professional Services Team based on your organization’s reporting needs.
What to Expect
After an injury is linked to an incident, the assigned EHS user must:
Review the injury record for completeness
Update or verify any missing details
Finalize the report once all required fields are complete
⚠️ Required fields are flagged automatically. Users should not finalize the report until these are addressed.
Reviewing Injury Record Sections
Injury records are divided into structured sections. EHS users should review each one carefully:
Personal & Contact Information
Injured party’s name, contact details, and employee type
Street address, job position, and hire date
💡Most of this is populated automatically if the person exists in the directory
Injury Details
Nature and Mechanism of injury
Part(s) of body affected
Description of the injury
Actions leading up to the event
Object or substance that caused harm
💡These fields play a direct role in regulatory reporting and OSHA logs.
Medical Treatment
Treatment provided (e.g., first aid, emergency care, hospitalization)
Medical facility and healthcare provider details
Regulatory Details
The Regulatory Classifications section helps determine whether the injury is:
Work-related
Recordable
Reportable
While the system makes an initial classification automatically based on form inputs, EHS staff can manually adjust these values.
🔎 Example: If the injury was initially classified as recordable but is later determined to be non-work-related, the recordable flag should be unchecked.
Lost Time Tracking
If the injury resulted in lost workdays or job restrictions/transfers, users must complete the Lost Time section on the right-hand side of the page.
Note: This panel may be collapsed by default. Open the side panel by clicking the clock icon on the right of the screen :
Enter the first recordable day and last recordable day date of restricted duty or time away
If these dates are not yet confirmed, toggle the entry to Estimated
Return later to finalize once accurate dates are known
💡 This data is used to calculate total Days Away and Days on Restriction, which appear on OSHA 300/300A reports.
Finalizing the Report
Once all required fields are complete:
Click “Finalize Report” to lock the record
This action prevents further edits and ensures the report is ready for internal tracking and external compliance use
