This article provides an overview of the three primary forms and instructions on how to generate them in the system.


What Does OSHA Require?

Under OSHA’s Recordkeeping Standard (29 CFR 1904), employers are required to maintain three key records:

  1. Form 301 – Injury and Illness Incident Report
    A detailed report describing the specifics of each injury or illness (employee, what happened, how it happened, etc.)

  2. Form 300 – Log of Work-Related Injuries and Illnesses
    A running log of recordable cases throughout the year, listing names (except for privacy cases), case details, and outcomes

  3. Form 300A – Summary of Work-Related Injuries and Illnesses
    A year-end summary of totals from the Form 300 that must be posted publicly each February through April

OSHA Form 301: Generated from the Injury Record

Users can generate OSHA Form 301 directly from any injury record.

To do so:

  1. Open the injury record

  2. Expand the Export to PDF button at the top right of the page

  3. Select Export OSHA Form 301

This PDF format includes all required OSHA fields and can be stored digitally or printed as needed.

🔒 For privacy cases, personally identifiable information (PII) is automatically redacted.


OSHA Forms 300 & 300A: Year-End Summary Reporting

To generate these forms:

  1. Navigate to any Establishment Record (e.g., San Diego – Production)

  2. Select the OSHA Reporting tab on the left-hand menu

  3. Use the Start a report button to begin the process

  4. Once the system compiles eligible recordable cases:

    • Click Download to export the PDF summary


🔐 Only users with the EHS Manager role can generate OSHA 300/300A forms.


Privacy Handling

If an injury is marked as a privacy case:

  • The employee’s name is omitted from the Form 300

  • Only authorized users can view sensitive details

  • OSHA logs remain compliant with redaction rules
