Managing Risk Assessment Templates

Risk Assessment Templates are the foundation of Serenity’s Risk Management application. They allow organizations to define and standardize multiple types of risk assessments that reflect the different operational needs of their EHS programs.

Templates ensure consistency by predefining key elements such as severity and likelihood ratings, risk formulas, included hazards, and access permissions. This structure makes it easy to roll out Job Hazard Analyses (JHAs), equipment risk reviews, and other specialized assessments across sites while maintaining a unified approach to measuring and managing risk.

Why Use Templates

Templates serve as reusable frameworks for conducting assessments. They eliminate repetitive setup work and ensure each team evaluates hazards and risks in a consistent, comparable way.

Organizations typically use different templates for different types of assessments — for example:

  • Job Hazard Analysis (JHA) – for task-based hazard assessments

  • Equipment Risk Assessment – for evaluating specific tools, machines, or systems

  • General Facility or Area Risk Assessments – for broad operational or environmental hazards

Although Serenity allows each template to use its own rating scales and risk formulas, most organizations benefit from keeping a standardized scoring model. Using consistent formulas for all risk assessments enables clearer cross-site reporting and better top-level risk visibility.

Creating a Risk Assessment Template

To create a new template:

  1. Navigate to the Risk Assessments module.

  2. Select the Assessment Templates tab at the top of the screen.

  3. Click Create new template.


A modal window will appear, prompting you to name the new template. Enter a clear and descriptive label, such as Equipment Risk Assessment, and confirm the Active checkbox is selected if you want the template to be immediately available.
Then click
Submit.

Once submitted, you’ll be taken to the new template’s setup page where you can configure all template properties.
