Reporting unsafe conditions, broken controls, or process gaps as soon as they're identified is one of the most important things anyone can do to support workplace safety. In Serenity, these observations are logged as Findings—task-based records used to track and resolve potential issues before they lead to harm.
Here’s how to report a Finding quickly and easily, right from the Serenity platform.
Report a Finding from the Homepage
For quick reporting, any employee can log a new Finding directly from the homepage.
Click the “Report an incident” dropdown in the upper left.
Select “Log a finding.”
This is the fastest way to report a safety issue, especially if you’ve just noticed something in the moment—like a tripping hazard, a blocked exit, or an equipment malfunction. It’s accessible to everyone, no matter their role.
Use a Custom Intake Form (If Enabled)
If your organization has enabled custom intake forms, you’ll also see these under the “See something? Say something!” section on the homepage.
These tailored forms allow for issue-specific intake—ideal if your company wants to collect certain types of data depending on the type of issue being reported (e.g., safety hazards vs. safety good catch).
Report a Finding During Other EHS Tasks
Findings don’t have to be logged as standalone reports—they’re often discovered during other EHS processes. You can create a Finding while working on:
An incident report
An investigation
A risk assessment
An inspection
This ensures that issues are captured in real time, without having to leave your current workflow.
The Finding Entry Experience: Simple and Streamlined
When logging a Finding, Serenity presents a quick-entry modal that focuses on just the fields needed to begin the resolution process—like a short description, priority, and location.
Once the record is created, users can view the full Finding record for additional details and hazard classification. From here, fields like hazard category, health impact, and scope of risk can be filled in to support prioritization and follow-up.
