  • Navigate to Risk Assessments in the Early Access menu within the More tab.

  • Tap + to start a new assessment and select a template (e.g., "Hazard Risk Assessment").

  • Select the Entity (Location, Asset, Vehicle) and assign the assessment to a group or user.

Hazard Analysis

Once the assessment is created, tap the Hazard Analysis tab. This is where you evaluate specific dangers (e.g., "Flying Debris" or "Shearing").

  • Applicability: Select a hazard category and mark it as Yes if it applies to the current situation.

  • Inherent Risk: Rate the risk level before any safety measures are applied. You will select the Severity (how bad it could be) and Likelihood (how often it might happen).

  • Control Plan: This is where you list safety measures.

    • Add Controls: You can manually add controls or tap Get AI suggestions to have the app recommend safety measures based on the hazard type.

  • Residual Risk: After adding controls, re-rate the Severity and Likelihood. Ideally, this score should be lower than your initial "Inherent Risk."

Mitigation & Activity

  • Mitigation Tab: If the risk is still too high, you can create new Corrective Actions or Findings directly from this screen to ensure the team fixes the issue.

  • Activity Tab: Use this to see a timeline of changes or add comments for coworkers.

Closing the Assessment

  • Tap Complete assessment.

  • The app will warn you if you missed any required items.

  • Enter your closing notes, draw your signature, and tap Sign and submit.

How to Build a Risk Assessment Template

This guide is for administrators or managers who need to create new risk assessment templates for the team to use.

Create the Template

  1. Navigate to the More tab (bottom right) and select Risk Assessments.

  2. Tap the Templates tab at the top of the screen.

  3. Tap the + button (bottom right) to create a new template.

  4. Label: Give your template a clear name (e.g., "Construction Risk Assessment").

  5. Active: Ensure the Active box is checked so your team can see it.

  6. Tap Save.

Add Hazards

Now that the template exists, you need to populate it with the specific hazards users should check for.

  1. Open your new template and scroll down to the Hazards list (or tap to add).

  2. Select Categories: You will see a list of hazard categories (e.g., Biological, Electrical, Ergonomic). Tap the dropdown arrows to expand them.

  3. Choose Hazards: Check the boxes next to the specific hazards relevant to this inspection (e.g., "Slippery Surfaces," "Pinch Point," "Falling Objects").

    • Note: If a hazard doesn't exist in the library, you can tap "Create a new hazard" to make a custom one.

  4. Tap Add or Save to confirm your selection.

Configure Score Settings

This step defines how the app calculates risk (the math behind the assessment).

  1. Inside the template, tap the Score settings tab.

  2. Risk Formula: Select how you want risk calculated.

    • Example: "2 Factors: Severity + Likelihood" (This adds the two scores together).

  3. Risk Criteria: Define the options users will see (e.g., "Rare to Almost Certain" for Likelihood).

  4. Risk Matrix:

    • Here you define what the final scores mean.

    • You can customize the levels (Low, Medium, High) and their associated colors.

    • Thresholds: You can input specific number ranges. For example, you can set it so that any score between 10 and 12 is considered "High Risk."

  5. Residual Risk: Scroll down to define how the app calculates risk after safety controls are applied (e.g., set specific calculation rules or allow manual entry).

  6. Tap Save.

Final Verification

Once your Hazards are added and your Score Settings are configured, your template is ready. You can test it by going back to the main Risk Assessments screen and tapping + to start a new assessment. Your new template will now appear in the list.
