The Incident Management module in Serenity is designed to help organizations capture, track, investigate, and report on workplace incidents with precision and ease. Whether you're managing near misses, injuries, illnesses, spills, property damage, or vehicle accidents, Serenity provides the tools needed to meet regulatory requirements and promote a culture of safety.

What is considered an Incident?

An EHS Incident is any event that has caused harm, or had the potential to cause harm. Examples of these types of events include:

Injuries & Illnesses
Near Misses
Chemical Spills or Releases
Property or Equipment Damage
Vehicle Collisions or Damage

These events can be reported by any system user, ensuring that employees at all levels can help maintain a safe work environment without requiring advanced permissions.

Incident Management Workflow in Serenity

Reporting Incidents

Incidents can be reported directly from the Serenity Home page using the “Report an Incident” button or a custom intake form designed specifically for your workforce.

Reviewing and Managing Incident Details

Once submitted, assigned users are notified and can begin reviewing the incident. Each incident may include related detailed reports:

Injury & Illness
Spill & Release
Property Damage
Vehicle Damage

EHS users must review and finalize these records before the incident can be closed. Fields required for OSHA compliance are flagged to help ensure complete documentation.

Investigating Incidents

For high-severity incidents or those involving injury/illness, a formal investigation can be launched directly from the incident record. This creates a separate task where investigators can:

Collect and review evidence
Perform root cause analysis
Assign and track corrective/preventive actions
Generate an investigation summary

This separation ensures clarity between the incident record and the investigation process.

Closing an Incident

Each organization sets its own closure policies. Some may close incidents after finalizing reports, while others keep them open until investigations or follow-up actions are completed.

To close an incident:

All associated reports must be finalized
Users can optionally request a review before closure
Reviewers receive an email notification to approve or request changes

OSHA Recordkeeping

Serenity supports OSHA Forms 300, 300A, and 301:

Form 301 can be generated from any injury record
Forms 300/300A are available under each Establishment’s OSHA Reporting tab

The system auto-calculates metrics like:

Total recordable incidents
Lost time and restricted days
DART, TRIR, and LTIFR

Users can download OSHA logs at any time, and Serenity ensures these reports reflect finalized and properly classified injury records.

Tracking Hours Worked

Accurate OSHA reporting and KPI calculation require monthly entry of hours worked:

Navigate to the Establishments Center
Click "New Time Cards"
Submit hours worked per establishment for the reporting period

Users can enter hours for multiple employee types (e.g., employees, contractors) to ensure precision.

For greater efficiency, Serenity offers integrations with payroll and timekeeping systems like ADP, Workday, and SAP. These integrations are available via our professional services team and help ensure your safety metrics and reports are always up-to-date.

Roles & Permissions

Serenity uses four licensed roles to control access:

EHS User – access to assigned incidents
EHS Manager – view all incidents and reassign tasks
Privacy Case Viewer – access to sensitive records
EHS Admin – full access plus configuration capabilities

Roles can be assigned to individuals or groups. Best practice recommends assigning them to groups for easier maintenance.

Dashboards & Insights

All incident and injury data feed into real-time dashboards, providing:

KPI tracking (TRIR, DART, LTIFR)
Injury type and mechanism breakdowns
Time-based trends (by day, week, month)
Heatmaps by establishment, position, or category

Dashboards give EHS leaders and executives the visibility they need to monitor safety performance and drive continuous improvement.
